Special Diet Statement

Special Diet Statements

Bluffview Montessori School follows the regulations in the National School Lunch Program Act for accommodating special diets. If your child needs special diet accommodations:

  • Lactose Intolerance

    • Bluffview Montessori will provide lactose-reduced or lactose-free milk as required by state law (MN Statutes section 124D.114) for children with lactose intolerance. Parents or guardian must make this request in writing using the Lactose Intolerance Form. A physician statement is not required for this substitution. We are not allowed to substitute for milk with juice or water unless we have a Special Diet Statement completed by your doctor.

  • Food Allergies

    • Food allergies that result in severe, life-threatening (anaphylactic) reactions meet the definition of a “disability” which requires that the substitutions prescribed by the licensed medical physician be followed.  The Special Diet Statement needs to be completed by a physician before accommodations are made.

    • Food intolerances and non-life threatening allergies do not result in a disability as written statement completed by a licensed physician.

These forms do not need to be completed on a yearly basis, but must reflect the child’s current needs. Bluffview Montessori does request a voluntary authorization with allows the school to discuss the special diet order with the physician.
